Job Summary: Under the supervision of the Medical Director, you will provide occupational therapy services to program participants. You will work closely with an interdisciplinary team to assess and develop care plans, provide treatments, and support staff and family members in executing therapy programs.
Key Responsibilities:
- Interdisciplinary Team Participation: Participate in team meetings, initial, semi-annual, unscheduled, and annual assessments; report changes in participants' status to appropriate staff daily.
- Quality and Process Improvement: Involved in the development and implementation of QAPI (Quality Assurance and Performance Improvement) activities.
- Screenings and Assessments: Perform intake screenings and comprehensive assessments at appropriate intervals, developing care plans for various settings, including homes, hospitals, and nursing facilities. Assessments include functional status, ADL (Activities of Daily Living) status, cognitive functioning, and home safety evaluations.
- Treatment and Care Planning: Provide appropriate treatments, develop and supervise therapy programs, and delegate tasks to other staff when necessary. Ensure safety measures are in place.
- Equipment and Training: Assess and order durable medical equipment, train and supervise staff on therapy programs, and educate on body mechanics and service availability.
- Documentation and Compliance: Complete necessary documentation and reports, maintain participant confidentiality, and adhere to organizational policies and procedures.
Qualifications:
- Minimum of one year of experience working with a frail or elderly population.
- Bachelor’s degree in occupational therapy from an accredited program.
- Current OT license in California.
- Valid driver’s license, clean driving record, and proof of auto insurance.
- Successful completion of pre-employment screenings and COVID-19 vaccination.
